Growth Mindset Collective - Participant Waiver and Release Form

Acknowledgment of Risks and Waiver of Liability

As the legal guardian of {name} ({dob}), residing at {address}, I hereby acknowledge and agree to the following terms as a condition of {first_name}'s participation in the Growth Mindset Collective Summer Camp:

1. Risk Acknowledgment: I understand that participating in jiu-jitsu and other physical activities conducted by the camp involves certain risks of physical injury. Despite precautions, accidents and injuries may occur. By signing this waiver form, I assume all risks related to the use of any and all spaces used by Growth Mindset Collective Summer Camp.

2. Waiver of Liability: I agree to waive and relinquish all claims I or my child may have as a result of participating in the program against Growth Mindset Collective and its instructors, employees, volunteers, agents, and all other persons or entities acting for them (hereinafter collectively referred to as “Releasees”), on behalf of myself and my child’s heirs, representatives, executors, administrators, and assigns.

3. Medical Treatment: In the case of an emergency, I permit the camp to provide necessary medical treatment for {first_name}. I also agree to be responsible for any medical or other charges in connection with my child’s attendance at the camp. Emergency contact: {contact_name}, {contact_relation}, {contact_phone}.

4. Media Release: I consent to the use of {first_name}'s photograph, voice, and video recording for use in any promotional materials, publications, or on the internet by Growth Mindset Collective Summer Camp and its affiliates. I acknowledge that participation in publications and websites produced by Growth Mindset Collective is voluntary and that my child will not receive financial compensation of any type associated with the taking or publication of these photographs or participation in company marketing materials and other publications. I further agree that participation in any publication and website produced by Growth Mindset Collective confers no rights of ownership whatsoever.

5. Indemnification: I agree to indemnify and defend the Releasees against all claims, causes of action, damages, judgments, costs or expenses, including attorney fees and other litigation costs, which may in any way arise from {first_name}'s participation in these activities.

By signing this document, I acknowledge that if anyone is hurt or property is damaged during {first_name}'s participation in this activity, I may be found by a court of law to have waived my right to maintain a lawsuit against the Releasees on the basis of any claim from which I have released them herein. I agree that this document is valid for subsequent visits and participation at Growth Mindset Collective Summer Camp without additional written authorization.
